Walker patients have the opportunity to receive care from an intraprofessional team from University of Minnesota including an on-site:
Geriatric physician
Certified nurse practitioner
and geriatric pharmacist
Walker Methodist’s holistic, patient-centered approach, combined with the medical expertise of the University of Minnesota, eases the transition process while providing quality care and service.
Our physician and nurse practitioner, along with the patient’s primary care physician, work closely to facilitate care and develop a discharge plan. Upon discharge, medical records and discharge plans are provided to your primary care physician.

Unlike many Transitional Care providers who are simply just one step in the process of returning home or home environment, Walker Methodist specialists are there for every step.
For many, whether their hospital stay was due to an accident, illness or elective surgery, the path to recovery can be a long and confusing journey.
We take pride in getting to know each one of our patients and understanding barriers to returning to a more healthy and active lifestyle.
Simply put, our staff gets to know you and your needs.
From there, we help manage communications between primary care physicians, nurse practitioners, care systems and managed care organizations. We tailor therapies to increase patient’s levels of independence and work in cooperation with your primary care provider so information is immediately accessible and schedule visits as medically necessary.
Our social workers and nurse managers work to provide tailored education for that final step from our transitional care unit to your home environment.
